Lash Intake Form
All new clients are required to complete a lash intake form upon arrival. Your safety and well-being are our top priorities, and this form allows our lash technicians to be aware of any potential allergies, sensitivities, or medical conditions that may impact the lash application process.
Deposit Requirement
A $20 deposit is required to secure your lash appointment. You can conveniently send the deposit through e-transfer to jasminenailslashbar@gmail.com. Our receptionist will reach out to you to confirm the receipt of your deposit, ensuring that your appointment is secured.
Cancellation/Reschedule Policy
As our lash technicians operate on a scheduled appointment basis, we kindly request that clients provide at least 24 hours' notice when canceling or rescheduling an appointment. This allows us to accommodate other clients who may be waiting for an available slot. Failure to provide the required notice will result in the retention of the deposit fee.
